COMORBIDITY

In order to facilitate research into the presence of more than one condition, several comorbidity data items were derived for the 2014-15 NHS using data from the ICD10-Classification.

The data items available are:

  • Comorbidity (Self report) (including Diabetes, Kidney disease, Cardiovascular disease)
  • Comorbidity (Self report) - Musculoskeletal
  • Comorbidity (Self report) - Chronic Lower Respiratory

For each comorbidity data item, the following section outlines the:
  • Definitions used to derive the data item
  • Population applicable to the data item
  • Methodology used to produce the data item.

For information on the ICD10 classifications or Condition status item, refer to the Health Conditions section of this Users' Guide.

Population


The population for the comorbidity data items is all persons in the 2014-15 NHS and they are located on the selected person level.

Methodology

Information about comorbidity was first published in the National Health Survey: First Results, 2014-15 based on a sample of 19,259 people.

COMORBIDITY (SELF REPORT)

Definition


This data item is produced for combinations of the following conditions:
  • Diabetes
  • Kidney disease
  • Cardiovascular disease (CVD).

For the purposes of deriving this item the following definitions were used:

Diabetes


To be classified as having diabetes, the respondent needed to have been diagnosed with diabetes mellitus (type1, type2 , type not known) regardless of their condition status.

Kidney Disease


To be classified as having kidney disease the respondent needed to have been diagnosed with kidney disease and have reported that the condition is current. This condition is assumed to be long-term and is identified as having condition status of 1,

Cardiovascular DIsease


To be classified as having cardiovascular disease (CVD), the respondent needed to have been diagnosed with one of the following conditions:

The following conditions were required to have been diagnosed and also current and long-term (condition status 1):
  • Hypertensive diseases
  • Tachycardia
  • Oedema
  • Diseases of arteries, arterioles & capillaries
  • Diseases of veins lymphatic vessels etc
  • Other diseases of circulatory system
  • Symptoms signs involving circulatory system

The following conditions only required diagnosis (condition status 1,2 or 3):
  • Ischaemic heart diseases
  • Other heart diseases
  • Stroke
  • Other Cerebrovascular diseases

Methodology


Using the defined conditions, the following output categories were produced:

1. Diabetes only
2. Kidney disease only
3. CVD only
4. Diabetes and kidney disease only
5. Diabetes and CVD only
6. Kidney disease and CVD only
7. Diabetes and kidney disease and CVD
8. Does not have Diabetes or kidney disease or CVD

In this item each respondent is allocated to only one category.

In order to create some flexibility with the use of this item, categories 1 to 3 were produced, which contain persons with only one condition and also category 8 which contain persons with none of the conditions.

To produce frequencies based on different combinations of these conditions, users can collapse categories as required. For example, to determine those people with either Kidney Disease or CVD, combine categories 2,3,4,5,6,7. To determine those people who don't have CVD, combined categories 1,2,4 and 8.

COMORBIDITY (SELF REPORT) - MUSCULOSKELETAL

Definition


This data item is produced from 2014-15 NHS data for combinations of the following conditions:
  • Arthritis
  • Other musculoskeletal conditions.

For the purposes of deriving this item the following definitions were used:

Arthritis


To be classified as having arthritis, the respondent needed to have current and long-term Rheumatoid, Osteoarthritis or Other/Type unknown Arthritis. These conditions are identified as having condition status of 1 or 4.

Other musculoskeletal conditions


To be classified as having other musculoskeletal conditions, the respondent needed to have a current and long-term status (condition status 1 or 4) for one or more of the following conditions:
  • Gout
  • Other arthropathies
  • Rheumatism
  • Other soft tissue disorders
  • Sciatica
  • Disc disorders
  • Back pain/problems nec
  • Curvature of the spine
  • Osteoporosis
  • Other diseases of musculoskeletal system & connective tissue
  • Symptoms, signs involving the musculoskeletal system & connective tissue.

Methodology


Using the defined conditions, the following output categories were produced:

1. Has arthritis only
2. Has other musculoskeletal conditions only
3. Has arthritis and at least one other musculoskeletal condition
4. Does not have arthritis or other musculoskeletal conditions

In this item each respondent is allocated to only one category.

In order to create some flexibility with the use of this item, categories 1 and 2 were produced, which contain persons with only one condition and also category 4 which contain persons who do not have a musculoskeletal condition.

COMORBIDITY (SELF REPORT) - RESPIRATORY

Definition


This data item is produced from 2014-15 NHS data for combinations of the following conditions:
  • Asthma
  • Other chronic lower respiratory disease.

For the purposes of deriving this item the following definitions were used:

Asthma


To be classified as having asthma, the respondent needed to have been diagnosed and the asthma to be current (has had symptoms in the last 12 months). This condition is identified as having condition status of 1 and 4.

Other chronic lower respiratory disease


To be classified as having other chronic lower respiratory disease, the respondent needed to have been diagnosed with bronchitis or emphysema or they had not been diagnosed but considered their condition to be current and long-term. These conditions are identified as having condition status of 1 and 4.

Methodology


Using the defined conditions, the following output categories were produced:

1. Has asthma only
2. Has other chronic lower respiratory disease only
3. Has asthma and at least one other chronic lower respiratory disease
4. Does not have asthma or other chronic lower respiratory diseases

In this item each respondent is allocated to only one category.

In order to create some flexibility with the use of this item, categories 1 and 2 were produced, which contain persons with only one condition and also category 4 which contain persons who do not have a respiratory condition.

Comparability with 2011-12

In 2011-12, the 'Comorbidity (Self Report) item was collected in both the NHS and the NNPAS surveys (i.e. the AHS Core), while the 'Comorbidity (Self Report) - Musculoskeletal' and 'Comorbidity (Self Report) - Respiratory' items were only collected in the 2011-12 NHS. In 2011-12, comorbidity items were calculated for both self-reported conditions and conditions based on biomedical data. As biomedical data was not collected in the 2014-15 NHS, comorbidity items based on biomedical data are not available.

When making comparisons between 2014-15 and 2011-12 data it is recommended that:
A number of changes have been made in the the 2014-15 NHS which may affect comparability of these items with 2011-12 data. These include:
  • Comorbidity (Self Report):
  • In the National Health Survey: First Results, 2014-15 , data on diabetes refers to persons who reported having been told by a doctor or nurse that they had diabetes, irrespective of whether the person considered their diabetes to be current or long-term. This definition was first used for estimates of diabetes in Australian Health Survey: Updated Results, 2011-12(cat. no. 4364.0.55.003). Estimates of diabetes for all years in the National Health Survey: First Results, 2014-15 are presented using this definition. In earlier publications, persons who had reported having diabetes but that it was not current were not included.
  • 'Heart, stroke and vascular disease' has been redefined to include persons who reported having ischaemic heart diseases and cerebrovascular diseases that were not current and long-term at the time of interview. Data for 2007-08, 2011-12 and 2014-15 in the National Health Survey: First Results, 2014-15 are presented using this definition, while previously published data excluded these persons.
  • In 2014-15, in addition to the existing category 'Stroke (including after effects of stroke)' on the prompt card for circulatory conditions, a new category 'Transient ischaemic attack (TIA, 'mini stroke')' was introduced and coded to 'Other cerebrovascular diseases'. As a result estimates of 'Other cerebrovascular diseases' have increased (from 4,900 people in 2011-12 to 171,200 people in 2014-15) while estimates of 'Stroke' have decreased (from 240,000 people to 172,300 people respectively).
  • Comorbidity (Self Report) - Musculoskeletal:
  • 'Back problems (dorsopathies)' have been redefined to include sciatica, disc disorders, back pain/problems not elsewhere classified and curvature of the spine. Data for all years presented in the National Health Survey: First Results, 2014-15 use this definition, while previous publications defined 'Back problems' as including only disc disorders and back pain/problems not elsewhere classified.
  • During processing of 2014-15 NHS data, an issue with coding of 'Back pain or back problems' in the 2011-12 NHS was identified in which some responses were incorrectly coded to 'Diseases of the digestive system' and 'Symptoms, signs and conditions not elsewhere classified'; analysis indicates that estimates of 'Back pain/problem, disc disorder' published for 2011-12 were under-reported by approximately 540,000 people, 'Diseases of the digestive system' were over-reported by approximately 250,000 people and 'Symptoms, signs and conditions not elsewhere classified' were over-reported by approximately 280,000 people. Data for 2011-12 for 'Back problems (dorsopathies)' are not comparable to other years. Data for 2014-15 have been correctly coded. ABS are investigating options to address this issue for previous years;

During processing of 2014-15 NHS data, an issue with coding of 'Gout' and 'Rheumatism' in the 2007-08 and 2011-12 NHS was identified in which a large number of people who reported having these conditions were erroneously allocated a status of 'current and long-term', resulting in over-estimation of prevalence in 2007-08 and 2011-12. In 2007-08 'Gout' was over-reported by approximately 800,000 people and 'Rheumatism' by approximately 290,000 persons, while in 2011-12 'Gout' was over-reported by approximately 280,000 people and 'Rheumatism' by approximately 145,000 people. Data for 2007-08 and 2011-12 for these two conditions are not comparable to other years. Data for 2014-15 have been correctly coded. ABS are investigating options to address this issue for previous years.
